Postoperative radiotherapy of squamous cell carcinomas of the oral cavity is still challenging and the level of evidence-based medicine in the field is sparce. This symposium will cover some of the burning questions like the significance of R1 margins and what levels to include both on the ipsi- and the contralateral side. Doses will also be discussed. Brachytherapy is a well-recognised modality in oral cavity cancer. Indications and techniques will be covered as well as the current evidence for using SBRT. The two modalities will be compared. Finally, concomitant treatment: when is it indicated and how as well as doses and types of drugs will be reviewed.

The session will focus on description, applications and perspectives provided by automation in EBRT. Autocontouring model’s training workflow as well as model generalizability will be presented. Performance evaluation measures and model monitoring in clinical practice will also be addressed. The physician view on automatic target delineation will be given with possible caveats. Automated treatment planning solutions and automation for adaptive online workflows will be described with their advantages and drawbacks. Furthermore, the use of automated solutions in LMIC will especially highlight the need of safe clinical implementation, training of professionals and guidelines.

The radiation therapy world moves towards modern approaches of big data analysis, automation, artificial intelligence, model-based treatment planning and multi-criteria optimisation. This symposium is meant to share current developments in these fields in brachytherapy with a strong link to clinical practice. A first focus is on large databases used for automated procedures in interventional radiotherapy with a glimpse on national and international regulations and quality assurance in data storage and usage. This is followed by how artificial intelligence can play a role in the general workflow of brachytherapy when it comes to pre-planning, delineation, reconstruction and planning (protocol- or example-based). We will discuss with examples of implemented multicriteria optimization for HDR prostate and cervix cancer brachytherapy. Detailed experience with the introduction of automated treatment planning will be shared with the main focus on its challenges and how to overcome them.

This session will focus on the actual challenges of patient positioning. The implementation of SGRT to daily clinical routine, based on the ESTRO Guidelines and an international survey on current practice will be presented. Patient preparation for MR-guided online adaptive SBRT for prostate cancer will be highlighted, including the differences in patient education and positioning between oART and conventional treatment. A research of an upright positioning system will be introduced. The first experimentations will be discussed by how the patient workflow were defined and how the immobilisation devices were designed. Also, the first clinical experiences will be shown in the pelvic and head and neck region.
