The European Particle Therapy Network (EPTN) became a task force of ESTRO in 2017. It was established in 2015 in response to the anticipated increase in the number of particle therapy centres in Europe. In addition, the need to cooperate among centres and integrate particles (i.e. protons and carbons) in the framework of clinical research networks was identified as being of paramount importance. ESTRO, at the time of initiation of the network, was asked to collaborate with EPTN and agreed to facilitate the group.

Particle therapy (PT) is only one part of radiation oncology, and needs to be well aligned with other radiation techniques as well as with general developments in cancer research and patient care. PT offers both new opportunities for providing excellence in cancer care, and for high-quality research within the framework of European  networks.

Organisation

The EPTN is led by three co-chairs and is composed of 8 working packages (WP) with experts of different topics to further elaborate the particle European discussions.
The EPTN meets once a year through the EPTN annual meeting.

Innovation is the key to the future of particle therapy. In the EPTN Innovation Workshop we will explore what is holding us back, what drives progress, and how we can move forward together. 

Particle therapy has a relatively slow innovation cycle compared to photon-based linear accelerators. This is partly due to the long lifespan of particle therapy equipment and the smaller size of the market. Yet innovation is more important than ever. It is key to improving patient outcomes, increasing efficiency, and ensuring that particle therapy remains accessible and sustainable for future generations of patients. 
This workshop brings together clinicians, physicists, researchers, industry representatives, and other stakeholders to explore how we can accelerate innovation in particle therapy and overcome the challenges that stand in the way. 

Together, we will discuss: 

  • The key barriers and enablers of innovation in radiotherapy 
  • Lessons learned from a successful proton therapy innovation journey 
  • What industry needs from EPTN to help accelerate progress 
  • The Medical Device Regulation (MDR): to be afraid or not? 
  • How co-creation and cost-effectiveness can maximize the impact of innovation 


The workshop will conclude with an interactive discussion focused on the central question: 
“How can we move particle therapy forward together through innovation?”

Clinical Indications for Online Adaptive Particle Therapy

Online adaptive particle therapy is still an emerging approach. Because it requires substantial additional resources, the field needs a clear clinical rationale: which patients, indications, or treatment strategies justify the extra complexity, and what new opportunities become possible only when adaptation is available at the treatment unit?

It is therefore essential to define which clinical scenarios and treatment concepts may truly benefit from online workflows, so that the field can prioritise those areas where the additional resources is most likely to translate into meaningful clinical benefit.

The value of online adaptive particle therapy should not be assessed only by asking whether adaptation improves plan quality on a given treatment day. Its broader value may be that it changes what can be safely attempted in particle therapy: simulation-free or direct-to-unit pathways, reduced margins, safer hypofractionation, adaptive dose escalation or de-escalation, response-guided treatment, and future biological personalisation.

This workshop will focus on identifying where online adaptive particle therapy matters clinically, where it may enable new treatment strategies, and which use cases should be prioritised

Mission

This work package is dedicated to dosimetry, instrumentation and quality assurance in radiotherapy with protons and light ions. It addresses the need for methods and harmonization in dose measurement across centers in Europe.

  • Disseminate and discuss reference dosimetry protocols suitable for particle therapy (e.g. IAEA TRS-398)
  • Facilitate inter-comparison exercises and dosimetry audits

Ongoing activities

  • Implementation of IAEA TRS-398 (2024 Edition)
    • Following the publication of the updated IAEA Code of Practice TRS-398, WP2 is leading efforts to interpret and apply the new recommendations in clinical settings.
    • A draft EPTN-WP2 Commentary summarizing clarifications, consensus points, and identified issues has been produced and will be published to this platform.
       
  • Intercomparison of Beam Monitor Calibration
    • A collaborative project aims to compare Gy/MU calibration curves across European centers.
    • The project will continue with standardized testing across multiple centers and the creation of a shared calibration database.
       
  • Development of PSQA Consensus (with WP5)
    • Building on the 2023 ESTRO Physics Workshop, WP2 and WP5 are co-developing a consensus on light ion Patient-Specific Quality Assurance.
    • Focus areas include:
      • Independent dose calculations
      • Log-file based QA
      • Risk analysis (FMEA)
      • External audits/surveys
    • Dedicated subgroups and follow-up meetings are ongoing, with industry interest in software integration for testing. ESTRO-EPTN Guidelines proposal have been accepted by the ESTRO guidelines committee and are being written.
       
  • Translating Out-of-Field Dosimetry to clinical practice for pregnant patients
    • Following the 6th WP2 Thematic Workshop held in March 2025 in Maastricht the organizers are defining follow-up activities based on the workshop’s outcomes.
    • Key areas of focus include:
      • Refining measurement protocols and dosimetric models
      • Drafting recommendations for clinical practice
      • Identifying research needs and standardization opportunities
    • Continued collaboration with EPTN WP4 and EURADOS is planned to expand the impact and reach of these efforts.
       
  • Validation of LET calculations
    • WP2 supports a new endeavor launched in 2022 by DCPT in collaboration with EURADOS WG9 for the evaluation of LET estimation techniques with experimental campaigns.
    • These efforts aim to assess the feasibility of LET reporting for treatment plan validation in clinical proton therapy.

TaSeRnet

The TaSeRnet project aims to harmonize proton therapy practice across Europe by building a robust network for RTTs and dosimetrists. The overarching goal is to unify and enhance the standards of proton therapy, ensuring high-quality proton therapy treatment for patients. Specific objectives are to: 1) identify best practices in which RTTs and dosimetrists play a leading role, 2) translate these best practices into  clinical guidelines and 3) create educational modules on advanced proton therapy tailored for RTTs and dosimetrists. Launched in January 2024 and running through December 2026, TaSeRnet is structured into several work packages coordinated by participating partners. These workpackages are outlined in the figure below.

Scope
The Adaptive Task Force brings together a multidisciplinary group of medical physicists, radiation oncologists and radiation therapists from proton and photon centres across Europe. The task force operates transversally across all EPTN Working Parties (WPs) with the goal of preparing the field for clinical adaptive proton therapy (APT).

The SWOT analysis was presented at ESTRO 2025 and manuscript is in revision.

Leads: Lorenzo Placidi (IT) & Katarzyna Czerska (CH)

Objective: To define and provide guidelines on commissioning and QA requirements for adaptive proton therapy.

4. Competence Matrix for Adaptive Workflows

Leads: Eva van Weerd (NL), Claudio Votta (IT), Rita Simões (UK)

Objective: To work towards establishing a robust RTT-led workflow model by identifying the competencies required for each workflow step and assigning the roles required for each competency.

5. Imaging Guidelines for Adaptive Workflows

Leads: Lena Nenoff (DE), Pietro Pisciotta (NL), Petra Trnková (CZ) and Jenneke Jacobs (NL)

Objective: to identify the Minimum imaging quality requirements for APT, as well as QA approaches for corrected/synthetic images.
