Purpose or Objective

Low dose rate brachytherapy (LDR BT) proved to be an effective organ-sparing method for patients with early penile cancer. The aim of our study was to evaluate results with high dose rate brachytherapy (HDR BT). It is the second largest study on this topic.

Material and Methods

Between 2002 and 2020 31 patients with early penile cancer were treated with interstitial HDR BT at the dose 18 x 3 Gy twice daily. A homemade brachytherapy template with stainless hollow needles or a 3D printed template with plastic tubes was used for tumour fixation.  


Results

With a median follow-up of 117.5 months (5-210), 8 patients developed a recurrence of penile cancer (25.8%). Seven of them were salvaged by partial amputation. Six patients died of internal comorbidities or another malignancy. Local control probability at 5 and 10 years was 80.7% (95%CI 63.7 – 97.7P%) and 68.3% (95%CI 44.0 – 92.6%), and cause-specific survival was 100%.  Only one case of radiation necrosis was observed. The probability of penis sparing at 5 and 10 years was 80.6% (95% CI  63.45 – 97,7%) and 62.1% (95%CI 34.8 – 89.4%) respectively.


Conclusion

Results of our study support the hypothesis, that HDR BT for penile cancer can achieve results comparable with LDR BT and allow organ sparing.
