Reflections from Porto: my ESTRO brachytherapy experience
My name is Leticia Alvarado, and I’m a radiation oncologist based in Guatemala City, Guatemala. I recently transitioned into active clinical practice. In June 2025, I had the privilege of attending this ESTRO brachytherapy course in Porto, Portugal. It’s a pleasure to share some highlights from this enriching experience, which proved both inspiring and professionally transformative.
As an early-career radiation oncologist practising in a developing country—where our specialty remains underrepresented and the demand spans multiple cancer sites—it has become essential to develop expertise in brachytherapy. My previous exposure to brachytherapy was focused primarily on cervical and prostate cancer, and I was eager to expand my clinical skills and deepen my understanding of this treatment modality across other anatomical sites.
The course delivered a comprehensive and multidisciplinary exploration of brachytherapy, blending theoretical foundations with hands-on training. Over five days, we examined a wide range of topics—from the physics of brachytherapy sources and radiobiological principles across low- and high-dose-rate modalities, to the clinical applications of techniques for the cervix, prostate, breast, head and neck, and skin. Practical sessions included contouring exercises, treatment planning, and applicator-based workshops, which offered experience with both interstitial and intracavitary approaches. Additional modules addressed uncertainties in brachytherapy, dose optimisation strategies, and radioprotection protocols, ensuring a well-rounded and clinically relevant curriculum.
One of the most valuable aspects of the course was the initial quiz, which helped me to evaluate my baseline knowledge and served as a catalyst for learning. I gained tremendously from the opportunity to engage with colleagues from diverse backgrounds and to share perspectives on how brachytherapy is implemented in different clinical settings. I particularly appreciated the chance to interact one-on-one with the course’s professors and experts, asking questions, clearing doubts from my daily practice, and receiving personalised feedback during contouring exercises. Case-based discussions were both instructive and collaborative, and I was able to garner practical advice on ways to improve dose distribution and implant quality, especially with limited resources. Beyond the academic scope, the course fostered genuine connections, networking, and friendships, and gave me the chance to explore the vibrant city of Porto.
I wholeheartedly recommend the ESTRO Comprehensive and Practical Brachytherapy Course to fellow radiation oncologists looking to enhance their skills and elevate their practice. The programme exceeded my expectations in both depth and delivery, offering scientifically rigorous and immediately applicable content. Its organisation was seamless, and the quality of faculty—internationally recognised experts—was exceptional. The continued access to course materials has proven invaluable, and the professional connections have opened doors for future collaboration. Coupled with the charm of Porto, this experience was not only educational but truly unforgettable. I’m already looking forward to the next opportunity to learn with ESTRO.
