The rapid evolution of systemic cancer therapies-including targeted agents, immunotherapies, antibody-drug conjugates, radioligand therapies, and novel endocrine and molecularly targeted treatments, has fundamentally transformed the therapeutic landscape of oncology. As these innovations become integrated into routine clinical practice, the role of radiation therapy is increasingly redefined within multimodal treatment strategies. Despite the growing use of combined systemic treatments and radiation therapy, robust guidance on their optimal integration remains limited. Key questions persist regarding treatment sequencing, timing, safety, toxicity profiles, biological interactions, and the identification of patients most likely to benefit from these combinations. The 2nd International Multidisciplinary Consensus on the Integration of Radiation Therapy with New Systemic Treatments, endorsed by the European Society for Radiotherapy and Oncology (ESTRO), aims to address these challenges through a structured, evidence-based and multidisciplinary consensus process. Bringing together international experts in radiation oncology, medical oncology, translational research, and patient advocacy, the meeting will focus on three major tumour domains, breast cancer, head and neck cancer, and genitourinary malignancies (prostate and kidney), to develop practical recommendations on the safe and effective integration of radiation therapy with modern systemic treatments.
